What is a cataract

A cataract is clouding of the eye’s natural lens. Vision becomes hazy, colours may appear faded and bright light can be uncomfortable. Most cataracts develop with age, although they can also follow an injury or occur alongside other conditions. An eye examination helps identify the cause of changes in your vision.

Cataracts also occur in younger people, and some are present from birth. Certain medicines or previous injuries may contribute to their development. Other eye conditions can cause similar symptoms, so an examination is needed to establish the diagnosis.

Cataract treatment

Modern cataract surgery

Replacing the cloudy lens

During surgery, the surgeon removes the cloudy lens and replaces it with an artificial intraocular lens. Planning includes an eye examination and calculating the lens power. Svit Ochey uses surgical systems from Alcon, Nidek and Oertli, and artificial lenses from Alcon and Johnson & Johnson. At your consultation, the doctor explains the treatment options and the outcome you can reasonably expect for your eye.

Laser assistance is one approach to individual stages of surgery; a phacoemulsification system helps remove lens fragments. Each technique requires appropriate equipment and indications. A technology’s name alone does not determine the outcome: eye health, planning and follow-up also matter.

Cataract removal methods

Ultrasound phacoemulsification

Ultrasound breaks up the cloudy lens so its fragments can be removed through a small incision. An artificial lens is then inserted. The surgical approach depends on the cataract and the health of the other structures of the eye.

Femtosecond laser-assisted active phacoemulsification

This approach combines laser-assisted steps with ultrasound lens removal. A femtosecond laser can create incisions, make an opening in the lens capsule and fragment the lens before removal. Planning uses measurements of the eye. The surgeon then removes the fragments and inserts an intraocular lens. The technique has indications and limitations; suitability and available treatment options are discussed at the consultation.

Torsional phacoemulsification

A variation of the ultrasound technique uses sideways tip movement to break up the lens. Equipment settings and surgical parameters are selected for the individual cataract. Your doctor will explain the technique planned for your procedure.

MICS — cataract removal through a micro-incision

Modern flexible intraocular lenses can be inserted folded through an injector. The lens unfolds inside the eye and takes the place of the natural lens. The incision size and whether stitches are needed depend on the surgical technique and procedure.

Discuss anaesthesia and preparation with your doctor before surgery. Tell them about your medicines, allergies and other health conditions. The doctor will explain how to prepare and which instructions to follow afterwards.

Modern artificial lenses

We use artificial lenses from Alcon and Johnson & Johnson (USA)

Vision after surgery depends on the health of the eye, the surgical approach and the artificial lens model. Discuss your work, reading, driving and other activities when choosing a lens. Different lenses offer different optical properties.

Monofocal lenses have one main focus. Your preferred viewing distance is discussed before surgery; glasses may still be needed for other tasks.

Toric lenses are designed to correct astigmatism. Multifocal models distribute light between several focal points. The choice depends on your eye health and needs; the doctor explains the benefits and limitations of the options.

A multifocal lens may reduce dependence on glasses at different distances, but it is not suitable for everyone. Discuss low-light vision and the possibility of glare around lights beforehand. Your doctor will help you weigh these characteristics against your expectations.

By the extent of lens clouding, cataracts may be described as:

  • early
  • immature
  • mature
  • hypermature

Cataracts are assessed by lens clouding and changes in vision. In the early stages, new glasses and better lighting may help with daily activities. When reading or other familiar activities become difficult, discuss surgery with your doctor. Eye drops do not make a cloudy lens clear again.

During surgery, the cloudy lens is broken up and removed, and a flexible intraocular lens is implanted in its place. Its optical power is calculated from the examination measurements. Final vision also depends on the retina, optic nerve and other structures of the eye.

Before the procedure, discuss preparation and any other health conditions. The doctor selects anaesthesia and explains the operation. The procedure length and when you can go home depend on your individual treatment plan. Vision recovers over time, so follow the prescribed care and attend follow-up examinations.

A cataract can develop in one or both eyes. If both need treatment, discuss the order of surgery and the interval with your surgeon. Each eye is assessed separately.

When should you have surgery? Timing depends on how much the cataract affects reading, work and your usual activities. The diagnosis alone does not determine the operation date. Your ophthalmologist assesses the changes and discusses timing with you; the lens does not have to become completely cloudy first.

Why remove a cataract

  1. Surgery replaces the cloudy lens with an artificial one.
  2. Timing depends on the examination and how the cataract affects everyday life.
  3. The ophthalmologist discusses the expected outcome, risks and lens options with you.
  4. Modern techniques allow lens removal through a small incision; the surgeon determines its size.
  5. The artificial lens stays in the eye. The need for glasses depends on the lens type and visual tasks.
  6. Follow-up examinations help monitor the eye as it recovers.
